I made this template some time ago as a favor for a lady who visited my blog. After emailing the finished product to her, I never heard a word back to say that she had received it, or a thank you for all the time and work I put into it. I have had it sitting on my pc for some time now, but it is too pretty not to be used somewhere. The set includes the background, all buttons and divider.
You would need to make your own header, but it's here free for anyone who wants it.
